I think there are two sentences above:
What have you been up to?(Enquiring what have you been doing recently)
Have you been visiting your friends?(in continuation suggeting whether you had been visiting your friends recently.)

The two questions are asking about the whereabouts of a person.
First question asks 'where have you been?'(as you were not visible recently), and the second question is in continuation of the first question, just like 'Whether you were visiting your friends as you were not visible/unavailable?'.

Just like you say, "You are not looking good, have you been sick lately?"

Since you are available now, the question is asking about the past, whether you were visiting friends in recent past, as you were not here.

I hate injections .. my mother is trying to take me to the clinic since morning but I know that they're ganna give an injection so I refuse I say:"I need to sleep .. I didn't sleep well yesterday " ..... kekekeke LOL
