"Pick someone you feel profoundly grateful ever decided to look your way." (From this video, 0:16-0:20)
Is this sentence grammatically correct? I think it should be something like this: "Pick someone that you feel profoundly grateful for and that has ever decided to look your way."

It's fine as it is. You are not asked to pick someone for whom you are grateful. You are grateful that they looked your way,

I can understand being glad somebody is my friend, but should I be profoundly grateful that somebody decided to look in my direction? I don't think so.
