An audio example sentence from Longman Advanced American English: Richards dismissed suggestions that he planned to resign.

I heard "plan." What did you hear, "planned" or "plan?"
 
They should be pronounced the same way.
 
The "d" of "planned" and the "t" of "to" run into each other, creating a single sound. That's why it sounds like "plan to" to you. If you really enunciate it (a native speaker is very unlikely to do so), you can make both sounds audible.
 
There is a little difference in both.
