6. My brother decided to fly to Rio de Janeiro last week because he got a free ticket.

This is my question: Instead of "Wouldn't have flown" can I put " wouldn't have decide to fly"?

Yes. It's a better answer (with my correction).
 
Thank you 5JJ. Just one more thing, I think the book has made a small mistake. You know where?
If you look at the instruction of the book exercise: "Rewrite each situation as a conditional sentence with mixed time frames."
You will see that the answer can/is not written in mixed time conditional. The answer key is not a mixed time conditional one.
Do you agree with me on that?
 
I don't think it's a particularly useful or clear instruction but there are two times involved - the decision to fly came after the receiving of the ticket.
