1) How frequently can we bill for this service?

2) What is the frequency allowed for this service?

3) How often can we bill this service(in a month or year)?

4) How many times can it be billed?

I don't understand why you need to ask that question at all. You already know the answer. You said in post #1 that it's just once a month.
