The key to this ex is A; then I wonder what's the difference between A and D? Can't I useou D?

I'm afraid it doesn't make much sense with any of the options.
 
Are you sure the "you" (Had you been) was not "she"?
Or perhaps the "she" was not "you"?

It's hard to see the connection between YOUR being careful on the exam, and HER results.

Oh, I'm so sorry. I made a typing mistake here. "she" should be "you". Then is there a better choice for the blank?
