Would you be kind enough to give me your considered opinion concerning the interpretation of the expression in bold in the following sentence?

Under Michal’s businesslike direction everything went off without a hitch. (W. S. Maugham, “Theatre”)

without a hitch = irreproachably, beyond reproach, like a house on fire, like one o’clock, smoothly

like one o’clock, sounds rather old-fashioned to me
smoothly, YES.
