Me to insurance representative: Why did you deny this claim?

But if I tell someone that I asked an insurance representative that why it was denied then what do we need to say? Do we need to say "I asked them why this claim denied"? Do we need to remove "did" because it is a statement rather than a question?
 
If you're talking about reported speech, use "I asked them why the claim was denied" or "I asked them why they denied the claim". As you can see, the word "did" doesn't appear in the reported speech versions.
 
Say:

But if I want to tell someone that I asked an insurance representative why the claim was denied, how should I say it?

Hm. Like that.

You switched from "I" to we in that sentence for no apparent reason.
