vitamins or vitamin tablet/capsule

Child: What's that on your mouth?

Mom:
1. It's my vitamins.
2. It's a vitamin tablet / capsule (if it's a capsule).

Which is more natural? I'm asking because I think the plurality of the word vitamins might confuse a child, seeing only one thing in the mother's mouth.
 
An American child would quite likely call that a vitamin.
 
Tell the child that the question should be 'What's that in your mouth' or 'What's that on your tongue?'

(It's a tablet — not a capsule.)
