To turn around in/on the bed

If someone lies in one position, but then changes to a different position, what would be the most appropriate way to say this?

He turned around in the bed.

or

He turned around on the bed.

Is there any difference in meaning? I prefer the first, because to me it means that he turned around while actually lying in bed, whereas the second means that he used his bed only as a platform to make the "turn around", so to speak. (This reasoning is perhaps entirely wrong, that's why I ask.)

I think you might want to say "He shifted in bed."

When I read yours, it made me think his head ended up where his feet had been.
"On the bed" would mean on the top of it, not under the covers.

When people change their position in bed, they sometime turn over.
