For the multiple choices on the examination papers. There are squares in front of the choices to select the answer.

Shall I say :

  1. please put a tick in the square
  2. please put a tick in the box
  3. please put a check in the box
  4. please put a check in the square

I would use #3. Please wait for a BrE-speaker to respond. I believe they will provide a different answer.

Yes - we'd use #1.

tick/check something

Is "tick or check something" more common than "put a tick or check in something"?

Definition and pronunciation of tick | Oxford Advanced Learners Dictionary
tick something
to put a mark (✓) next to an item on a list, an answer, etc
Please tick the appropriate box.
Tick ‘yes’ or ‘no’ to each question.
I've ticked the names of the people who have paid.

Check the box next to the right answer.

Yes, simply say "check the box."

Or 'tick the box'.

The box should be a little, empty box. There should not be any content in it.

What I mean is to check whether the answer is wrong or right?

These are usually some form of multiple choice. It's pretty self-evident what one should do. Saying "Please put a check in the box" seems to be giving more instruction than should be necessary.

I saw the title of this thread and thought it said "tick check". Here in the southern US, a tick check is what you do after taking a walk in the woods.

You need to explain this better.
The box is small. It has no content. You option about content would not work at all.

There should be a box next to several choices.

The thing you need to be clear about is whether they should check all the boxes that are correct, or check only one box for each question, or what.

For example:
Place a check (or tick) in the box next to each correctly written sentence.
or
For each question, check the box next to the correct answer.
