Is this correct? Since I mention the To address, do I need to mention To whom it may concern?

This is to certify that MR.......is working in our organization in the Capacity of GENERAL MANAGER since xxxxxxxx. He is drawing a monthly salary as mentioned below:

Other Allowances:

Gross Salary:

This certification is issued upon the request of Mr........ without any liability or additional responsibility to (company name)

Yes, use "to whom it may concern" if the addressee is not known.
I would write "has been working in this company as... .since ...".

If you want to use "in the capacity of", don't capitalise "capacity". Also, you capitalised "MR". That's not how it should be written. Capitalise just the first letter of "Mr" and the first letter of the surname.
If you don't know the name of the addressee, use "To whom it may concern" (make sure to capitalise "To").
