"Somebody help me" is an imperative sentence, asking for help.

this was just an example , I wanna know if after sb or sth , our verbs accompany with "S" or not .

The first is possible but unlikely as a complete sentence.

Yes, but it is impossible to say what is correct without a context.

I am going to stand here until somebody helps me.
Somebody, help me open this window.

Yesterday, somebody helped me feed the cows.
Can somebody help me with my homework?

Note that this is the Simple Present tense. If someone is helping you now, you say, "Someone is helping me" (Present progressive), not the simple present.
Now that you realise the importance of context, you should be able to supply some next time.

You are new here, so I will explain a few things to you.
1. Use capital letters.
2. Don't use words like "wanna" or "gonna" here.
3. Don'e leave a space before a punctuation mark. Do leave one after (as you have done).
4. Don't use abbreviations like "sb" or "sth" - they don't mean anything to some of us.
5. Join two sentences with period (or a semi-colon, or sometimes a dash), not a comma.
