4 and 5 are not possible, the others are all possible in the proper context.

1. I'd say 'blow on blow' is acceptable in the given context.

2. 'The boxers traded blow for blow.'

3. 'Her gave us a blow by blow account of the proceedings.'

I really don't understand the sentence.

'He received blow after blow' means 'He was hit many times'.

In my opinion, without any other words in the sentence, none of them make sense. I would say that only "after" could be inserted in the space for such a short sentence.
