Making good questions for Examination

1)I often got difficulties in making questions for examination. students thought that my questions were too difficult. they often got bad score. I cannot predict whether the questions is difficult or easy for the students. I just want to make them always study hard on English examination. therefore they will not under estimate my lesson. what should I do as a good teacher in making questions?how to make good questions?
