What's the difference in meaning if I remove the underlined "it is"?

Lots of people ask unclear or poorly articulated questions. It's vital to clarify what it is they want to know, or, if you are uncertain, why the questioner is asking that particular question.

There is no real difference in meaning without "it is".
 
You could claim that it is makes it more emphatic, but I agree with emsr2d2.
