Is he doing better?/Has he gotten better?

Girl: How is grandpa? Is he doing better?/Has he gotten better?

Dad: No. He's not doing so good.

Girl: Isn't he going to wake up?

Dad: I don't know yet.

If the grandpa is in a coma, what would be the most natural question to ask? Would any of my two suggestions be perfect or is there a better one? Thank you.
 
Perhaps:

Alice: How is Grandpa? Is he getting any better?
Ben: No, he's not doing well at all.
Alice:: Is he going to wake up?
Ben: I don't know.
 
"Is he doing better?" is natural. "Has there been any improvement?" is another option.

There is some potential ambiguity in English when saying someone has "gotten better." I would understand it to mean a full recovery.

"I was sick, but now I'm better" means I am no longer sick at all.
 
Note that British English speakers usually say 'got' and not 'gotten'.
