Kindly, be informed about my leave for the month of October
I would be on vacation with my family starting from 12[SUP]th[/SUP] Oct to 16[SUP]th[/SUP] Oct.

It's not appropriate to address your manager with "Hi."
Consider, e.g.,

Dear Mrs Jones,
or
Dear Dr Flynn,
or
Dear Jim, [If you're on first-name terms with your manager]
 
In AmE, would normally say from the 12th of Oct. through the 16th of Oct.
 
In BrE, we don't use "through" that way. To make it clear that the holiday dates are to include October 16th, we would say "Please note/consider my application for leave from October 12th to October 16th inclusive".
