Are these sentences OK?

If he had driven slowly, he wouldn’t had been killed in the road accident many years ago.
If he had driven slowly many years ago, he wouldn’t had been killed in the road accident.
 
I prefer the first.
 
"If he had driven slowly, he wouldn’t have been killed in the road accident many years ago."
 
I agree with Tdol.
But the main clause should be corrected to "... he wouldn't have been killed in ..."
