3) Where are you headed towards?

4) Where are you heading towards?

Do all these sentences mean the same?

We say these to ask someone where they are going. Am I correct?

Yes, and yes.

When it's a wh- question word, "where" includes the idea of direction; you could replace it with "in what direction". Including "towards" is redundant.
