1. Do you ever sleep in class?
2. Have you ever slept in class?
3. Did you ever sleep in class?
What's the difference in meaning between the above sentences>
 
The first two questions are for a student (or teacher!) who is currently studying, whereas the third question is rather for someone who is no longer studying.
 
I'd say "Have you ever fallen asleep in class?" Using "sleep/slept" makes it sound as if the person did it on purpose.
