Could you please correct my letter request for leave

Greetings! I was given five (5) days service leave June 17, 2016. In this regard may I request that this service leave be consumed by me worthily on June 12 to16, 2017 which is the period prior to the giving birth of my wife. As soon as my wife’s tummy gets bigger her movements become limited and it gets harder for her to attend to the needs of our three year old child, thus, I would like to spend this leave to take charge in the taking care of our daughter to spare her from getting stressed and give comfort while preparing for her delivery on the succeeding days of my leave and also to ensure that if anything unwanted happens in relations to her pregnancy I will just be right beside her to help.

Dear Ms Matheson,

I'd like to change the start date of my five days' leave from 17 June 2017 to 12 June 2107. This is because my wife is due to give birth around that date, and I will need to be available for that and to look after our three-year-old daughter while my wife is in hospital.

Please reschedule my leave and confirm.
