If the form was almost correct, I would have done it. But there's errors in most lines.

1. Have you visited Czech Republic? If yes, how long did you stay?
a) No, I don’t know Czech Republic. You may have visited Czech Republic and still not know it. We don't use 'know' like that.
b) No, but I know Czech Republic. See above

Neither of those first two answers is right.
