Which is correct?

(1) cost a mere a dollar
(2) cost mere a dollar
(3) cost a mere dollar

I would choose (3). You can also say: cost merely a dollar.
Why don't you give the full sentences?

Actually, the sentence I saw was "It cost a mere four dollars." I just changed the number.
 
It might be easier just to ask the question you want the answer to in your first post.
In fact you didn't change the number. If you had, the sentence would go, "It cost a mere one dollar."
