Does 'be in good shape' mean being healthy or is it to do with someone's appearance?

it usually means 'in good physical condition'.

In good shape is a phrase that, depending on context, can be used to affirm physical fitness, or financial soundness, or emotional well-being, or any similar measure of optimal functioning. The idiom can also apply to the condition of inanimate objects.
