Does it mean "I hope you are doing well."

I can answer:

I am fine.
I'm good.
I'm ok.
Not too bad.
Just fine.

1. Are the above answers correct?
2. "Just fine" means it's not very good. Is it right?
3. "Not too bad" is the same as "I'm ok". Is it right?

"I'm just fine" is an unlikely response but, if spoken evenly with no pauses or stress, it would be taken to mean "I'm absolutely fine". However, if you said "I'm just ... fine", the "just" would be taken to mean "only" or "just about" so it wouldn't be as enthusiastic or optimistic a response.
