How many syllables are there in this word?
a-ppre-ciate
or
a-ppre-ci-ate?

Four, the second one.

Is it so everywhere or are there any exceptions?

There's four everywhere.
Also in: negotiate, depreciate, conciliate, etc.
I can't think of any (English) '-iate' words where this suffix is not pronounced as two syllables.
