Even the most formidable thinkers speak to us out of lives pretty much like our own, with their daily routines, their little aches and pains and pleasures, and their occasional upheavals.(From The Deepest Human Life by Scott Samuelson)

Does “little aches and pains and pleasures” mean "little (aches and pains) and little pleasures" or "little aches and little (pains and pleasures)"

What does it mean?

 
Daily routines bring little aches and pains — that is, minor discomforts — and little pleasures.
